Upping Items

Hi All,

This is going to seem like a really silly question.....

There's a recipe for "upping" unique items from Normal -> exceptional, and exceptional -> elite.

But after looking around for an hour, I cant seem to find the recipe. Could someone point me in the same direction?
(I tend to get all of my info from www.diabloii.net)
