Some questions regarding HOWL and BERSERK

1 - HOWL : Does it work on bosses/champions/superuniques/etc... ?
2 - HOWL : What is the condition for this skill to work (cLvl > mLvl ???)
3 - BERSERK : Does it work with Crushing Blow ?
4 - BERSERK : Does it work with Prevent Monster Heal ?
