"Twinked" means you have a reasonable supply of items (or you can trade for them) to use.
"Untwinked" means you char only uses what he finds.
As such, the "twinked" setup is usually the better one, although it often has stuff that you can't trade for. So, view them as suggestions, and guidelines for mods that you want to look out for in your gear.

I don't know whether there are any builds that only "allow" one choice of equipment, but Skelemancers certainly aren't like that.
