Are you talking about something item specific, like charms or white/grey items?
Obtaining an item with an Ilvl of 96, 97, and 98 is very much possible. Imbueing, item rewards, gambling, shopping, certain horadric cube recipes, etc. can all generate items with an Ilvl of 96 - 98.

ok ok true enough. I should have been more clear. When a monster drops an item, what are the possible ilvls? Nil is 95 and he is the highest besides Baal (in terms of monsters that actually drop things). My main question is can a monster drop something with an ilvl of 92 and 93?

Oh, in that case, no 89 - 93 (at least not in 1.10+)... I think the next step from Diablo would be Ilvl **.

** = Unique monsters in mlvl 85 areas
92 = Nihlathak
94 = Diablo
99 = Baal
I can't think of any monster with an mlvl of 89, 90, 91, 93, 95, 96, 97 or 98.
Maffer (and the other council members in his area) are mlvl 86. Nihlathak is also mlvl 92 and not mlvl 95.

About Nihlathak, based on memory:
Nihlathak is a monster with Mlvl 92, the Nihlathak you fight is a boss version of Nihlathak, which is why he gets the +3 bonus. So monstats and ATMA are correct.
80 - ** is all there, AFAIK.

Err. aye. I was looking at his base mlvl in monstats which is 92. Oddly enough, ATMA lists him as mlvl 92 as well.

ATMA might have that problem of forgetting the +3 boss bonus with respect to all super-uniques of that kind, those who aren't just a boss of an existing monster type, but a special monster with its own graphics etc. Other examples: Griswold, the Smith, the Summoner, Hephaistos.

I thought item rewards were fixed ilvl, based upon the quest and the difficulty.
