It has to be 3 different Keys. One Terror, one Hate, and one Destruction. Transmute in Hell Act 5.

Just to add. As you know, the Terror key drops from the countess on hell. The hate key drops from the summoner, and the destruction key drops from Nihlathak. The higher player count, the better the chance of a key dropping.
You need one of each key to open a portal in act 5 to one of the first uber boss areas (either Uber Izual, Uber Duriel or Lilith). There's no way to guarantee which portal will open, but only one instance of each can be opened in a game, so it's best to open all three in the same game. This means having nine keys total (3 Terror Keys, 3 Hate keys and 3 Destruction Keys).
After the first three uber bosses are killed, transmute the 3 organs that they dropped in act 5 to gain access to Uber Trist. You don't need to open this portal in the same game as the first three though, so you can wait until later on if you wanted to.
