How does life leeching work? anyone can explain for me in depth?

I have a lvl 24 amazon wearing two Cathan rings each gives +6% life per hit, and using a magical javalin which gives +5% Life per hit, that is total of 17% per hit. Currently this Zon has life of 300, so each hit should give me near 50 points of life. that is obviously not the case... I got something like 10points each hit, more or less.

so what is going on?

It leeches a percentage of the damage you deal, independently of your total maximum life.

Moreover, there are penalties on LL in nightmare and even bigger penalties in hell.

Moreover each monster has a leech effectivness property from 0 to 1, which is in fact multiplier. From monster with leech effectivness 0 (undead skeletons for example), you don't leech at all.

Moreover, it only leeches from physical damage you deal and it's also affected by physical damage resistance of particular monster. If monster is 90% resistant against physical damage, you deal only 10% of your normal damage and you get also less life leeched back.
