any area with lots of monsters you can kill quickly (eg act3 jungle, cows)
LK is also good with all the chests/poppables

Hmm, this seems like a good thread for a first-post-of-all-time. (I am a rather late joiner to these forums, I know!)
In my personal experience, I find that one of the quickest ways to get flawless gems is from Baal's Minions (the five waves in the throneroom).
For added efficiency, you can join public Baalrun games where someone teleports to the throneroom. As people in these runs are usually only interested in experience points or items, almost no one picks up gems, leaving them all for you.
You also get good experience as a side effect! :thumbup:

Definitely Lower Kurast Super Chests. In an hour you can fill your stash with flawless and perfect gems. I also just scored a 5 resists all +18 life sc.

I'm currently running LK in Hell, on Battle.net. Not so much now, but on ocassion, when I'm having a dry streak with my usual MF runs.
Tons of charms, gems and a few runes pop out of many of the LK chests if you've got the time and patience to crack open a couple hundred.
Quick note: In 7-8 player games these chests seem to emit a lot more loot. If you want to raid LK while people are Baal running it's doable.
A few raisins why running LK in Hell is better than the Tombs Nightmare: No FE to worry about, no cold immunes beyond uniques, skillers can drop, better runes can drop.

If all you're interested in is flawless gems, my suggestion would be NM cows. They drop flawless with very reasonable regularity, and they die a lot faster, especially if you have a good crowd-clearing AoE attack like Frozen Orb, Lightning Fury, etc. Harder to get 8 player games since you cant see public NM games, but if you just talk about gems per unit time, I think its the best yield.

I agree with everyone here, these are all good suggestions. Just pick whatever you can kill the fastest without losing interest. I'd also like to suggest NM arcane sanctuary. With all the ghosts, and super chests, it's not too difficult to get dozens of gems in an hour, if you join full games.

As far as the Arcane Sanctuary, one problem there is that with the ghosts able to fly into empty space, if you kill them when they are not on the platforms, they don't drop anything. So the Arcane Sanctuary isn't as good a choice for area-of-effect killers like Blizz sorcs. The River of Flame has the same problem, with drops falling into the lava instead of empty space.
If you truly want to optimize gem yields, you can build a Classic LF zon and have her run NM cows. With there being no charms, jewels or runes to drop, the overall rate for gems is noticeably higher. You could then convert the gems with mules to LoD. But I don't see that being worth the trouble! People used to do similar things with 1.09, where chipped gems were unusually useful for the reroll recipe there. A standard method was a Clasic chipadin, who was a Holy Fire paladin who harvested chipped gems in A1 normal for mules to convert them to LoD.

Regarding the drop chance per monster, ghosts in nightmare and hell are best (apparitions, gloams, souls, storm casters etc.), but I guess at the time you can find and kill one ghost, you can kill more than 2 nightmare cows (which have about 1/2 of the drop chance of the top end ghosts) which are my recommendation if it's just about flawless gems. Also, Councils have a very high chance to drop flawless gems and it's always worth to visit 9 monsters of that kind if they are just next to a waypoint, no matter if on nightmare or on hell mode.
