It doesn't even have to be a high lvl, 1 point in IM is all a necro needs to mess you up. You will take big damage if you only hit once, the 2nd usually kills you.

i dont know about a revive (should be the same as golem)
I dueled a necro that said "I bet you cant win a duel against my golem" you cant back down against that it seems so easy. he puts up a clay golem and right as i start to smite he put IM on me. Well, i died instantly. (and i had like 4.5k life.) i know it works with fighting a golem so it should work against a revive.

It works with any non-player minion.
And it does not matter what kind of life you have, if you just have a high enough damage, then it will go ahead and beat you up right in the face. Kinda like if you hit yourself in the face: you are only going to be hit as hard as you hit, only as many times as you hit and only as fast as you hit.
However, no matter what minion or how much damage, Life Tap > Iron Maiden, always. This is because unlike Iron Maiden, Life Tap is not further reduced by your own PDR% and PvP penalty so it's always much, much greater than Iron Maiden will ever be, specially since the amount it heals is close to the amount of the damage that Iron Maiden returns.

Life Tap on a minion with Iron Maiden is a Full Rejuvenation Potion.
Only if half of your damage is the same as your full life or half of your damage is equal or greater to the amount of life you are missing from a full life status.
