Ilvl = Clvl +5, with these caps in normal:
    Act 1: 12
    Act 2: 20
    Act 3: 28
    Act 4: 36
    Act 5: 45

    Normally Ilvl 41+.

The general principle is that the ilvl is inherited from the mlvl (the level of the monster which dropped the item).

The mlvl itself is generally inherited from the area level (alvl). This is true for normal monsters, in nightmare and in hell. Super uniques and bosses have their own mlvl (alvl+something for SUs).

I leave it to more expert people to give you the details. Alternatively you can search for a thread by Thrugg in this forum, in the title the word "levels" was present, this gave a very comprehensive view of all levels in the game.
Sorry not to look for it myself, but I'm a bit in a hurry now.

I now had time to find that thread. It is:
    http://forums.rpgforums.net/showthread.php?t=268215

The word "levels" is not present...

and another mistake: my apologies to Gta-maloy] for having wrongly remembered of his work as being Thrugg's. But Thrugg's posts are always so good and accurate  that it is not too much an offense. ;)
