Since no one actually bother to post useful information as of yet.
Following is skill placement I use for my PvM Werewolf build.
20 Lycanthropy
20 Werewolf Form
20 Fury
20 in either Oak Sage or Heart of Wolverine depends if you are struggling to hit monsters or dying too much. Oak Sage gives you more Life while HoW gives you more attack rating and damage.
rest of the points really up to you. I would suggest Feral Rage and Grizzly Bear summon.
Stats will depend on what equipment you have or able to get. General rule is enough strength and dexerity to use equipment and rest into vitality.
If your not so rich in terms of items following gear is optimal but worthwhile to use.
Weapon: Immortal King Stone Crusher with 2 IAS jewels socketed, Buriza socketed with your choice (I would use Amn rune), or Hune Sudan with 3 shaels socketed (I believe that how many needed to reach 4 fpa with fury).
Armor: Shaftstop, Duress runeword, or Duriell's Shell.
Helm: Jalala's Mane, Vampire Gaze, or Tal Rasha's.
Belt: String of Ears (best bang for price imho)
Boots: Aldurs, Natalyas, Immortal Kings, or any other boots with 40 Faster R/W.
Gloves: Bloodfist, Venom Grip, or any IAS set glove (Sanders, Laying of Hands, Magnus).
Rings: Raven Frost, Drawf Star, Life Leech, or Angelic (if you use Amulet).
Amulets: +Skills to Druid with nice stats or Angelic (if you use rings).
*note on Angelic ring and Amulet. This combo gives you bonus stats to AR and Life.
