The item level (ilvl) for +2 to all skills of a certain class must be 90 which is also the affix with the highest level, with respect to amulets. When crafting, it depends on the character level (clvl) as well: new ilvl = ilvl/2 + clvl/2 with both summands being rounded down before adding them. That means if you have an amulet from a L85 monster (e.g. a regular monster from the Pits), you have to be L96 to have a chance of getting +2 to skills.

When gambling amulets, they are up to 5 levels below your character level. You can also make use of the fact that a gambled +2 to skills amulet is L90 at least. That means a L90 character who crafts a +2 to skills amulet into a new one, has a chance to get a +2 one again. If you are L93 or higher, any gambled amulet can be used, not just those which already have proven to be L90. In that case, the amulet has a level of at least ** and **/2 + 93/2 = 90.

The simple answer:

+2 to a tree is not the same as +2 to a character class.

The long answer:
For a certain affix to be possible on an item:

  • alvl >= affix_lvl
For amulets specifically (this does not hold for other items:
  • alvl = ilvl
The crafting recipe sets the items ilvl as so:
  • [clvl/2] + [input_ilvl/2] = new_ilvl.
Putting these 3 together, we see that for a certain affix to appear on a crafted amulet:
  • [clvl/2] + [input_ilvl/2] >= affix_lvl
We know from here, that:
  • +2 character class affix_lvl = 90
  • +2 curses affix_lvl = 40
So to craft an amulet with +2 character class:
  • [clvl/2] + [input_ilvl/2] >=90
and to craft an amulet with +2 curses:
  • [clvl/2] + [input_ilvl/2] >=40
On a lvl 77 character, any amulet with ilvl > 4 can be crafted into a +2 curses amulet.

I hope this helps a little.

alvl just means maximum affix level. I was trying to distinguish between alvl as a property of an item and the level of each affix.

It is not true that any amulet of level 4+ can have +2 to a tree.
Any amulet with item level of 4+ can be CRAFTED BY A LEVEL 77 CHARACTER to have +2 curses...or any valid tree. (+2 bow & crossbow, javelin & spear, and barb combat do not appear on amulets.)

In Nightmare, I think ilvls are based on mlvls...you can check out the mlvls at d2data.net. Of course, you can get ilvl 85 items from "The Pit" in A1 Hell, if you can handle the monsters there, which is the same ilvl as you'd get from A5 hell world keep. If all you're after is +2 tree rares, though, it doesn't matter where you get them from at your clvl, so long as it isn't A1 norm.

If you really want to craft quality amulets, and have a chance of getting an uber +4 ammy (+2 class, +2 tree...just hope they're both for the same class if you're ever lucky enough to roll both prefix groups together), you'll want to reach Clvl 93+ and craft gambled ammys. (at clvl **, you'd have a 10% chance to get the ilvl required, at 92 you'd have 90% chance, in-between each level makes a big difference)

Of course, you could already get a +1 class, +2 tree ammy, which would also be highly valued, if it has a couple other decent mods.

Edit: noticed you said "nightmare", corrected for relevance

My bad on saying +2 tree and +2 class skills can stack...can't get a +1 class/+2 tree either...thought I'd seen a barb helm that had the two together, but I was remembering wrong.
