You will release 3+1 per 5lvl of CS bolts.
Number of bolts that hits enemy is based on the size of monster. large monsters as Urdars, Pit lords etc will be hit by all bolts. Medium monsters will be hit by around half the bolts. Small monsters (as devilkins, flayers) will be hit by around quarter of bolts.
For small and medium monsters it depends also on direction from where you make your attacks. I try to attack them directly from North or South to maximize the number of bolts that can hit.
