Lvl 20 I'd say. I've never had TS on a character, so I'm not sure, but it works that way for other buffs like BO. >_>

I think the lightning mastery bonus stays, and I'm pretty sure the lvl of thunderstorm will go back down again after you switch equipment.

You couldn't pre-buff it in 1.09. I haven't tried it in 1.1 though , so can't answer it there.
