Sorry if any of these have been posted, it's quite an extensive list you guys have made, and I didn't read it all yet. Anyway, here are some with names:

of the Unscarred: X (or X%) More damage if health is above Y (or Y%)
of the Quake: Walking around small monsters can knock them back, stun them, etc. for a short period of time. Must be wearing boots and/or heavy armor
Frustrated: Increases damage, decreases defense for a short period of time (i.e. Berserk) after a miss
Trickster's: Turns any extra health orbs (i.e. after you're fully healed and there are no monsters within X distance of you) into bombs which explode when an enemy walks on them (Maybe this could be a skill?)
Astral: increases melee attacks' range up to a certain amount.
Shatterbone's: %Chance to instantly kill an undead, undead explodes on impact damaging any nearby enemies or allies. (Or just %chance to explode on death and damage nearby anything)
ninja's/of the ninja: +damage in darkness (or +defense in darkness)
And some without names:
-Character leaves behind acid/flaming/frozen footprits, which poison/burn/freeze monsters that walk into them
-Reveals the position of hidden doors/destructible environments/hidden treasure chests
-for Shields: after blocking an attack, X% chance of making the enemy's weapon less dangerous (so imagine a spiked shield that tears enemy weapons apart)
-for Shields: X% chance to knock back any enemy whose attack you just blocked (animation should take as long as the usual blocking animation)

%chance of corpse explosion after slaying a monster
%chance of disarming and enemy for X seconds
%chance of blinding an enemy for X seconds
%chance of rooting and enemy for X seconds
%chance of Silencing caster for X seconds

I also think that unique items should start at level 1 and gain experience as the player does until that item is level 10. The unique items will start with a few mods such as enhance dmg and a stat bonus. As the item levels up it gains its other stats/abilities.
For Example: Windforce
Starts with:220-300% enhance damage
+15Dexterity
At level 2 Gains:30% increased attack speed
At level 4 Gains:7-10% Life Leech
At level 6 Gains:30%Open Wounds
At level 8 Gains:30% Slow Enemies
At level10 Gains:%chance to cast level 15 tornados



Also i think items should have usable abilities. A picture of your armor or weapon could be dragged down to your UI Hotbar. when clicking the picture of your item an ability is activated. However, in order to do this, these usable abilities must have cooldown timers.
For Example:War Travelers
/use ability could increase a players run speed by 45% for 8-10seconds
When used these abilities should be recognizable by other players in the form of an Aura, Weapon color/effect. etc.

Summon swarm on hit, summon a swarm of little birds or even bugs, like locust swarm, on hit. Similar to poisoin damage or bleeding, but it just looks cooler

NEW TRIGGERS:
X% chance to do something when casting a spell
X% chance to do something when an enemy casts a spell
X% chance to do something when an ally dies


In the affixes below, "attacking" could be repalced by other triggers as well.

-of thawing: X% chance to remove cold effects when attacking
-of antidote: X% chance to remove all poison effects when attacking.
-of cleansing: X% chance to remove all curse effects when attacking.
-of omnipotence: X% chance to remove all curse, poison and cold effects when attacking.
-of freezing: X% chance to displace a cold effect to a nearby enemy when attacking.
-of poisoning: X% chance to displace a poison effect to a nearby enemy when attacking.
-of cursing: X% chance to displace a curse effect to a nearby enemy when attacking.

I'll add some. no idea if any of these are posted yet

Items could have some random penalty too. Either inverse ine of the benefiting affix/suffix or do totally new ones... Cursed stats
Chance when hitting enemy to give it part of your mana
Chance when hitting enemy to get an infection from it causing you to randomly daze every few seconds for a minute.
Chance on hit to get curse from enemy to lower your dmg by 50% for short period of time

Spirit: Increase mana regeneration by 2-10% when mana below 20%
Executioner: Ignore defences when enemy has under 10% health
Soothe: Hits destroy 1 fury from target
Lucky Strike: Triple experience from monster if it dies of a crit
Escape Artist: Increase movement speed by 2-10% when below 10% health
Fiery Weapon: 2-10% chance to add [massive amount] of fire dmg on hit
Regeneration: Reduce maximum life by 2-10% but regenerate 1-5% of maximum health/second
Masochism: Reduce mana regeneration by 2-10% but cause 3-15% damage taken to go mana
Spell Power: Increase spell dmg by 2-10% when mana above 75%
Revenge: Increase melee dmg by 2-10% when victim of crit
