attacker takes dmg question

say for example you have attacker takes dmg of 5... does that mean exactly what it says...

.... so if for example the monster has 100hp and he attacks does this mean he will go straight to 95hp and so on?

and does the attacker actually have to hit you for this to take effect?

I'm not sure, I think it's more of a factor of 5, because it would be useless in nm and hell.
Someone can explain it better then me. And probably any succesfull attack triggers it

It does exactly 5 physical damage to the attacker when he hits you. And yes, it's useless in NM and Hell.
