Hephasto is demonic armorer, found near the Hellforge in the River of Flame. He must be killed to obtain the Hellforge Hammer, which is required to smash Mephisto's Soulstone and complete the Hellforge quest. Hephasto shares graphics with the Smith from Act One, and they're both modeled in homage to The Butcher, from Diablo I.

Prior to v1.08, Hephasto was one of the most dangerous monsters in the game, thanks to the fact that he was always Cursed and Extra Strong. One hit kills to Hephasto were not uncommon for many types of characters, and for this he was especially dreaded by hardcore players. Hephasto with Extra Fast was as bad or worse than Multishot Lightning Enchanted, in those early days, since he would run you down and end you, while at least an MSLE could be avoided.

Hephasto was nerfed in v1.08, and he now spawns with Conviction, Spectral Hit, and Magic Resistant, making him somewhat difficult, but nothing like the killer he was in the early days of Diablo II.
